Medications at Day Camp

We cannot accept any medications at camp that are not accompanied by the appropriate form.

Severe Allergy Health Care Plan- If your child has a severe allergy and carries an epi-pen, we need this form completed by a doctor.

Respiratory Health Care Plan- If your child has asthma and carries an inhaler, we need this form completed by a doctor. 

Other Medication- If you child needs to take other medications during the day (including prescription drugs, OTC drugs and vitamins), this form must be completed by a doctor. 

All forms can be brought to the first day of camp.
